GENERAL MEDICAL SERVICES

In addition to the usual medical services including health promotion, chronic disease management, immunisations, family planning and child health, the practice offers Enhanced Services such as Near Patient Testing (we monitor patients taking such drugs as warfarin and methotrexate) and Minor Surgical Procedures (including joint and soft tissue injections and the removal of skin lumps).

The practice is also an approved yellow fever vaccination centre.

NEW PATIENTS

Please call in at our main surgery in Chirk to register. You will be asked to complete a questionnaire about your medical history. It is important that we gather as much information as we possibly can about your health as this helps our doctors and nurses in making medical decisions about you before we receive your clinical notes from your previous surgery which can often take up to a couple of months.

All new patients over 5 years of age will be offered a new patient medical check which will usually be carried out by one of our nursing team. If you are on certain medication, or have a chronic medical condition, a GP will see you.

REPEAT PRESCRIPTIONS

Repeat prescriptions are issued at the doctors' discretion and are normally for patients on long term treatment.

All repeat prescriptions are computerised and the safest way to order one is to tick the required items on the request slip attached to the prescription. Please hand the slip into reception. Please allow two working days for us to process your request.

You may now order your repeat prescription online. Please go to the Online Services page.

If you wish us to post your prescription to you, you must enclose a stamped addressed envelope. You will be asked to see a doctor every 6 or 12 months to ensure that your treatment is still appropriate.

TELEPHONE ENQUIRIES

For routine enquiries, including the results of tests, please telephone Reception on 01691 772 434 between 11.00 a.m. and 4.00 p.m. when it is less busy. If you need to speak to a doctor the receptionists may ask you to call back later so as not to interrupt a consultation. The doctors are usually available to speak to you between 11.00 and 11.30 a.m. For reasons of confidentiality please ring in person for test results. Receptionists are not medically trained so they may only tell you if a result is normal or that you need to make an appointment to see a doctor.

NON NHS SERVICES

The doctors are happy to carry out medical examinations for such purposes as insurance, sports medicals and certain types of driving licence. They will also fill out certain forms such as fitness to travel certificates. The NHS does not cover these services so a charge will be made; details of current charges are available on this website or at Reception.

FLU VACCINATION

Immunisation against influenza is recommended for patients with asthma, emphysema, heart, or kidney problems, diabetes, immune system difficulties, carers, those living in nursing homes and all those over 65. Please contact Reception in late September to make an appointment.

TRAVEL IMMUNISATION

Please make an appointment at least eight weeks in advance of your departure to ensure adequate cover if immunisation is required. A charge will be made for certain immunisations not covered by the NHS. Please ask at Reception if you would like a list of these charges.

NHS DIRECT

NHS Direct (0845 46 47)
NHS Direct is a nurse-led confidential 24 hour medical service that you can call for immediate advice on what to do if you or a family member need advice on particular health conditions or medications.

OUT OF HOURS CALLS

Out of Hours medical care is now the responsibility of the Betsi Cadwaladr University Health Board who have contracted the service to the North East Wales Out of Hours Service (NEWDOC). When the Surgery is closed you should ring

You will be answered by a trained call handler who will take basic details from you and arrange for your needs to be assessed by a health professional. You will be given advice and may be asked to attend a primary care treatment centre or receive a home visit if it is necessary.

N.B. If a patient's life seems at risk call an ambulance (999) as well as NEWDOC.

PRACTICE FACILITIES

Chirk (01691 772 434)
Chirk is our principal surgery housed in well-modernised eighteenth century buildings which are now Grade II listed by the Welsh Office. There is access for wheelchairs and toilet facilities for the disabled. Some of the consulting rooms are upstairs , but if you are unable to climb stairs your doctor will arrange to see you on the ground floor. There is a hearing loop at reception.

St. Martins (01691 773 334)
If you live in St. Martins, Dudleston Heath or Gobowen you may choose to attend the branch surgery at St. Martins.

Patients of this surgery may obtain prescribed medicines from its dispensary. You must register as a dispensing patient - the Receptionists will tell you how to do this. The dispensary is open during surgery hours.

There is access for wheelchairs and toilet facilities for the disabled. There is a hearing loop at reception.

Please note that calls are diverted to the Chirk Surgery when there are no staff at St. Martins.

Weston Rhyn
If you live in Weston Rhyn or Selattyn you may choose to attend the surgery which is held in the Institute. Surgery is held on Monday and Thursday at 9.00 a.m. and there is no appointment system. Facilities are currently rather limited at this surgery so we ask that you attend at Chirk whenever possible.

There is access for wheelchairs and toilet facilities for the disabled.

Chirk Community Hospital (01691 772 430)
We look after patients at the Community Hospital and staff the minor casualty service. There are Physiotherapy, Occupational Therapy, and Day Hospital facilities available to patients of the Practice. X-ray facilities are currently suspended and under review.

Special Equipment Fund
The generosity of patients and relatives has enabled us to purchase items of medical equipment to improve our services to the community. Donations are always welcomed and gratefully acknowledged.

DISABLED ACCESS

There is access for wheelchair users and toilet facilities for the disabled at all three surgeries. There are hearing loops at Chirk and St Martins.

Chirk presents a number of problems. There is very limited parking on Castle Road The nature of the building precludes the installation of a lift so doctors see patients with mobility problems on the ground floor. St Martins has easier access and we have marked a parking space for disabled patients. Weston Rhyn has difficulties because the doctor works alone with no reception or staff support. Parking, access and toilet facilities are good. Staff are receiving ongoing training in order to support patients with disabilities and will always be ready to help. We are exploring ways of making our practice information more easily available to patients with disabilities, whatever the medium.

ACCESS to RECORDS

Patients may access their own medical records, written and electronic, by a written request to the Practice Manager. Records may not be moved from practice premises and a charge may be made for copies.

Community Nurses and Health Visitors have access to the records of patients in their care.

Access to people outside the practice team is only ever given with the patient's express written permission.
